July 1, 2009 see's the start of the new US Club Soccer season and teams will need to renew their player passes and adult background checks. This page lists the simple steps needed to renew:

Part 1. Exsisting US Club Players, Coaches and Adult Volunteers
Team Manager or Coach to email NYSA Registrar with the names of those returning players and supply:

1. 1" x 1.25" photo per player - please WRITE NAME on back of each one or supply photo album

2. Coaches and managers should undertake the online background check (no forms required) (at start choose "Club" and then Novato from the dropdown menu)

Nb. your are responsible for the retention of medical release forms which never need renewing. The NYSA does NOT retain a copy.

Part 2. New Players, Coaches and carded Volunteers

Team manager or coach to supply:
1. Completed (and signed in two places) US Club Medical Release form (click link), which will be returned to you. PLEASE ENSURE YOU COLLECT EMAIL ADDRESS IF AT ALL POSSIBLE.
2. Adults should undertake the online background check (at start choose "Club" and then Novato from the dropdown menu)
3. A copy of players birth certificate
4. 1' x 1.25" photo
